Medigap in California: Medicare Supplement Plans 2026

If you have Original Medicare and are tired of high copayments, coinsurance and deductibles, Medigap could be the solution you are looking for. In California, supplemental Medigap plans offer financial protection and more flexibility in choosing doctors.

This complete guide explains everything you need to know about Medigap in California: how the plans work, what each covers, how much they cost, your guaranteed rights and how to avoid costly mistakes.

What is Medigap and How Does It Work

Medigap, also known as Medicare supplemental insurance, is a private insurance plan designed to complement Original Medicare (Parts A and B). Medigap helps pay the costs that Original Medicare does not cover, such as copayments, coinsurance and deductibles.

How Medigap Works

Here is the simple process:

  1. You keep Original Medicare (Parts A and B)
  2. You buy an additional Medigap plan from a private insurer
  3. When you visit the doctor, Original Medicare pays first
  4. Then Medigap pays the remaining shared costs (if covered by your plan)
  5. You pay the monthly Medigap premium plus any costs your plan does not cover

Important point: Medigap does NOT include prescription drug coverage. If you need drug coverage, you must enroll in a Part D plan separately.

Why Choose Medigap

Many people choose Medigap because:

  • Maximum flexibility: You can see any doctor who accepts Medicare without network restrictions
  • Financial protection: No surprises from unexpected copayments if you choose a comprehensive plan
  • Peace of mind: Some Medigap plans have no out-of-pocket spending limits
  • Travel: Coverage nationwide anywhere in the United States
  • Security: If your health changes, you cannot be denied coverage (under certain circumstances)

Plans A-N in California

There are 10 standardized Medigap plans available in California: A, B, C, D, F, G, K, L, M and N. Each plan offers a different level of coverage, and all plans of the same type offer the same benefits regardless of the insurance company.

Description of Each Medigap Plan

Plan A is the most basic plan. Covers: Part B coinsurance (20%), copayment for emergency care abroad (80% after deductible), and first blood services abroad.

Plan B is similar to Plan A but also covers the Part A deductible (entire hospital deductible).

Plan C is one of the most popular plans. Covers: Part A deductible, Part A coinsurance, Part B copayment, Part B deductible, skilled nursing care copayment, foreign coverage and blood.

Plan D is similar to Plan C but covers up to 50% of Medicare dental and vision services through specific providers (does not cover complex dental procedures).

Plan F is the most comprehensive plan available. Covers all costs that Original Medicare does not cover, including both deductibles. However, it is no longer available for new enrollments if you enrolled in Part B after January 1, 2020.

Plan G is currently the most popular plan in California because it covers almost everything Plan F covers, except the Part B deductible ($257 in 2026). For this reason, it is significantly less expensive than Plan F.

Plan K is a lower-cost plan. You pay 100% of expenses until you reach an annual out-of-pocket spending limit (in 2026, approximately $6,700). After reaching that limit, Medigap covers 100% of covered expenses.

Plan L is similar to Plan K but with a lower annual out-of-pocket spending limit (approximately $3,350 in 2026), meaning better protection but higher premium.

Plan M covers 50% of the Part A deductible and 100% of Part A coinsurance. Usually has a moderate premium.

Plan N is an economical plan that covers most costs but you pay $20-$50 copayments for doctor visits and $100-$200 for emergencies. It is a good option if you want to save on monthly premiums.

Medigap vs Medicare Advantage: Which to Choose

The decision between Medigap and Medicare Advantage is personal and depends on your individual needs. Here is the key comparison:

Advantages of Medigap

  • Total flexibility to choose doctors and hospitals
  • National coverage without network restrictions
  • No cost surprises (if you choose a comprehensive plan)
  • Guaranteed protection against health changes
  • Many plans with no out-of-pocket spending limits
  • Greater freedom of choice

Advantages of Medicare Advantage

  • Monthly premium of $0 on many plans
  • Additional benefits included (vision, dental, hearing)
  • Integrated prescription drug coverage
  • Maximum out-of-pocket spending limit (financial protection)
  • Wellness services included
  • Good if you travel within the network

Medigap is better if: You travel frequently, have specific doctors you want to see without restrictions, or prefer maximum flexibility even if you pay a higher premium.

Medicare Advantage is better if: Your budget is limited, you need dental or vision coverage, or you are willing to use a closed network of doctors.

Guaranteed Rights in California

California offers special protections for Medigap beneficiaries that do not exist in all states.

Guaranteed Purchase Right (6 Months)

If you turn 65 and immediately enroll in Medicare Part B, you have 6 months (called the "guaranteed purchase period") to buy any Medigap plan without worrying about your medical history. During this period, insurers cannot reject you, impose pre-existing condition exclusions, or charge a higher premium for health reasons.

This period is critical because it is the only time you have the right to buy Medigap without health restrictions. If you wait beyond this period, insurers can deny you coverage or charge you more.

Birthday Rule in California (Protection 75+)

California has a special law called the "Birthday Rule" that protects Californians over 75 years old. Each year, during the month of your birthday, you can buy or change to a different Medigap plan without worrying about your health, and the insurer cannot reject you.

This is important protection because after age 75, pre-existing health conditions often result in rejections or higher premiums. The Birthday Rule ensures you always have an annual opportunity to change plans.

Plan Change Protection

If you already have a Medigap plan and want to change to a different plan, conditions depend on whether you are within the guaranteed purchase period or if the Birthday Rule applies. Outside these periods, insurers can deny you coverage if you have pre-existing conditions.

Best Medigap Insurers in California

Although all Medigap plans of the same type offer the same benefits, prices can vary significantly between insurers. The most popular insurers in California include:

  • United Healthcare (AARP): One of the largest, offers all plans, known for price stability
  • Cigna: Wide availability, good competitive prices
  • Anthem Blue Cross: Strong presence in California, good plan options
  • Humana: Competitive prices, good customer service reputation
  • Medico Health Plan: Regional insurer with good California prices
  • Omni Insurance Company: Competitive prices for certain plans

We recommend comparing prices from multiple insurers before choosing. Prices can differ by hundreds of dollars per year for the same plan.

Medigap Costs in California 2026

The cost of Medigap varies significantly depending on the plan you choose and your age. Here are approximate price ranges for California in 2026:

  • Plan A: $100-$150 per month (cheapest plan)
  • Plan B: $130-$170 per month
  • Plan C: $150-$200 per month
  • Plan D: $160-$210 per month
  • Plan G: $220-$280 per month (most popular)
  • Plan K: $80-$120 per month (with out-of-pocket limit)
  • Plan L: $140-$180 per month
  • Plan M: $100-$150 per month
  • Plan N: $130-$170 per month (with copayments)

Important note: These prices increase with age. At age 75, expect to pay approximately 40-50% more than at age 65.

How Prices Are Set

Insurers can use three pricing methods in California:

  1. Attained Age Pricing: Your premium increases each year you age (most common method)
  2. Issue Age Pricing: Your premium is based on your age when you first enrolled (increases slowly)
  3. Community Pricing: All customers pay the same price regardless of age (very rare in California)

When to Buy Medigap

The time you buy Medigap is critical because it affects your future rights and costs.

Optimal Time to Buy

Within 6 months of turning 65 and enrolling in Part B: This is the best time. You have the guaranteed purchase right without health questions. Insurers cannot reject you or charge you a higher price.

If you are 65+ but did not enroll in Part B 6 months ago: You can still buy, but insurers can ask health questions and reject you if you have pre-existing conditions. If rejected, your only options may be higher-cost plans or Medicare Advantage.

During Birthday Rule (California, age 75+): You can change plans during your birthday month without health concerns.

What You Should Do Now

  1. Determine if you are within your guaranteed purchase period (first 6 months)
  2. Compare plans based on your health needs
  3. Get quotes from multiple insurers
  4. Make sure you do not have a duplicate Medigap plan (one is enough)
  5. Review prices annually and change if you find something cheaper

Common Mistakes to Avoid

We have seen many people make costly mistakes with Medigap. Here are the most common ones:

Mistake 1: Waiting Too Long to Buy Medigap

If you wait beyond 6 months after enrolling in Part B, you lose your guaranteed purchase right. Insurers can reject you if you have health problems like diabetes or heart disease. If several plans reject you, the only option left is Medicare Advantage, which may not be what you want.

Action: If you are turning 65 soon, buy Medigap within 6 months of enrolling in Part B.

Mistake 2: Not Understanding Plan Differences

Many people buy the first plan they find without understanding what it covers. Some plans leave the beneficiary paying thousands in out-of-pocket expenses. It is important to understand exactly what costs your plan covers.

Action: Read the benefit comparison guide (called "Comparison of Benefits") to know exactly what is covered.

Mistake 3: Buying Multiple Medigap Plans

You do not need more than one Medigap plan. Insurers will not pay more just because you have two plans. It is wasted money.

Action: Review your current insurance policies. If you have multiple Medigap plans, cancel all except one.

Mistake 4: Not Reviewing Annual Price Changes

Medigap prices change each year, and insurers often increase significantly. Other insurers may offer better prices for the same plan.

Action: Each year (especially in October), compare your current plan's prices with other plans of the same type from different insurers. If you find something cheaper, consider switching if you are within your guaranteed purchase period or Birthday Rule.

Mistake 5: Not Understanding the Difference between Medigap and Medicare Advantage

Some beneficiaries buy Medigap without considering Medicare Advantage, or vice versa. Both options have pros and cons, and the best depends on your personal situation.

Action: Read our Medigap vs Medicare Advantage comparison and carefully consider both options.

Mistake 6: Forgetting to Enroll in Part D

Medigap does not include prescription drug coverage. If you do not enroll in Part D, you will pay a penalty and prescription drug costs can be very high.

Action: Make sure to enroll in a Part D plan separately, even if you are not taking medications now. You can change plans each year during the open enrollment period (October to December).
