Medicare Costs 2026: Complete Guide to Premiums and Deductibles

How much does Medicare really cost? This is one of the most important questions Medicare beneficiaries ask. And the answer is: it depends. Medicare costs can vary enormously depending on which type of plan you choose, your income and your health status.

This complete guide breaks down all Medicare costs in 2026, including premiums, deductibles, coinsurance, out-of-pocket limits, and how to find financial assistance if you qualify. We will make the numbers make sense.

Part A Costs (Hospitalization) 2026

Part A covers hospitalizations, skilled nursing facility care, hospice and some home care services.

Part A Premium 2026

For most people age 65 and older, Part A is FREE if you or your spouse paid Medicare taxes for at least 10 years (40 quarters).

If you did not pay these taxes, the premium in 2026 is:

  • Less than 30 quarters: $503 per month
  • 30-39 quarters: $281 per month

Part A Deductible and Copayments 2026

Although the premium is free, there are important out-of-pocket costs:

  • Hospital deductible: $1,728 per benefit period (all days 1-60 are your responsibility)
  • Coinsurance (days 61-90): $432 per day
  • Coinsurance (days 91+): $864 per day
  • Skilled nursing facility care (days 1-20): $0
  • Skilled nursing facility care (days 21-100): $216 per day
  • Skilled nursing facility care (day 101+): Your full responsibility

Important note: A "benefit period" begins the day you enter a hospital or skilled nursing facility and ends after 60 days without hospitalization.

Part B Costs (Medical Services) 2026

Part B covers doctor services, ambulances, durable medical equipment, lab tests, preventive services and more.

Standard Part B Premium 2026

The standard Part B premium in 2026 is $185.00 per month for most beneficiaries who enrolled in 2022.

Important: The premium is based on your enrollment year, so if you enrolled in a different year, your premium may be different (usually higher if you enrolled more recently).

IRMAA: High Income Adjustment

If your income is high, you will pay a higher premium. This is called IRMAA (Income-Related Monthly Adjustment Amount).

For 2026, if your income is above $97,000 (individual) or $194,000 (married), you pay more. For example:

  • Less than $97,000: $185/month (standard premium)
  • $97,000-$123,000: $259/month (additional charge of $74)
  • $123,000-$153,000: $333/month (additional charge of $148)
  • $153,000-$183,000: $407/month (additional charge of $222)
  • Over $183,000: $481/month (additional charge of $296)

Incomes used are based on your income from 2 years prior (2024 for 2026).

Part B Deductible 2026

The annual Part B deductible in 2026 is $257. Once you pay this, Part B covers 80% of approved services and you pay 20% (coinsurance).

Part B Coinsurance

After paying the deductible, you typically pay:

  • Doctor visits: 20% of approved amount
  • Outpatient services: 20% of approved amount
  • Lab tests: 0% (preventive) or 20% (diagnostic)
  • Durable medical equipment: 20% of approved amount

Part D Costs (Medications) 2026

Part D covers prescription medications. There are four coverage stages that affect what you pay.

Stage 1: Deductible

Part D plans have deductibles ranging from $0 to approximately $505 in 2026. You pay 100% of your medications until you meet the deductible.

Stage 2: Initial Coverage

After paying the deductible, until you spend approximately $4,500 in medication costs (pharmacy price), you pay 25% of most generic drugs and 25% of brand name drugs.

Stage 3: Coverage Gap (Donut Hole)

When your expenses reach approximately $4,500 up to $7,050, you enter the "coverage gap". In this stage, Part D covers 75% of generic medications and 50% of brand name drugs. You pay 25% and 50% respectively.

Stage 4: Catastrophic Coverage

After spending approximately $7,050 out-of-pocket, you enter catastrophic coverage. Part D covers 95% and you pay 5% for medications.

Average Part D Premium

The average Part D premium is approximately $35 to $50 per month in 2026, though it varies by plan and location.

IRMAA affects both Part B and Part D if your income is high.

How to Calculate IRMAA

  1. Medicare reviews your income from 2 years ago (2024 for 2026)
